DO YOU TAKE INSURANCE?

Kim is an "in network" insurance provider for Illinois Blue Cross/Blue Shield PPO and BCBS Blue Choice plans. As a contracted provider with your insurance company, Kim has agreed to accept their “reasonable amount” payment for services and you are not responsible for any difference in the amount billed and the amount paid by the insurance company. You will only be responsible for any co-pays, coinsurance, or deductibles as outlined in your plan.

HOW DO I VERIFY MY INSURANCE BENEFITS?

If you would like to utilize your managed health care plan, please call your insurance’s customer service line and ask for Outpatient Mental/Behavioral Health. Inquire as to the following:

  • What is the benefit eligibility for in-network and out-of-network providers?
  • What is the co-payment or co-insurance amount?
  • Do you have an annual deductible? With a deductible, although your insurance may cover a percentage of the costs, you may have to pay a certain amount out-of-pocket until you meet your annual deductible.
  • What is the percentage paid after the deductible is met?
  • Is pre-certification or authorization required? If so, Kim can discuss this with you more in detail.

WHAT IF YOU DON’T ACCEPT MY INSURANCE OR I CHOOSE NOT TO UTILIZE MY MENTAL HEALTH BENEFITS?

You can pay a self-pay, out of pocket fee, if you choose not to use your insurance benefits or if Kim is not an in-network provider. Payment is expected at the time of service.  Kim is an "out of network" provider for other types of insurance. If you are out of network, she will work with your insurance company to assess eligibility and facilitate partial reimbursement directly from your health insurance company in accordance with your policy using the Health Insurance Claim form.

WHAT PAYMENTS ARE ACCEPTED?

You can pay by cash, personal check, or credit card. You are also able to utilize a Flexible Spending Account (FSA) debit card for payment.
